fix(package-parser): visibility of function/parameters in re-exported classes (#630)

* fix: don't remove internal usages, so we don't add remove annotations for them

* fix: add 0 usages for all classes/functions/parameters

* fix(package-parser): properly compute visibility of function/parameters in re-exported classes
